HIPAA Notice of Privacy Practices

This notice describes how medical and health information may be used and disclosed by Denizen and how you can access that information. Please review it carefully.

Our commitment

Denizen respects the confidentiality of your and your family's health information. Where we handle protected health information (PHI) as defined by the Health Insurance Portability and Accountability Act of 1996 (HIPAA), we do so in accordance with HIPAA and applicable state privacy laws.

Our role

Denizen is not itself a healthcare provider. In most cases, Denizen functions as a business associate of participating home care agencies and healthcare organizations. In some cases, we may operate as a covered entity under HIPAA. In either capacity, we take our obligations seriously.

Safeguards

We maintain administrative, physical, and technical safeguards designed to protect the confidentiality, integrity, and availability of PHI. These include encryption of data in transit and at rest, access controls based on business need, workforce training, business associate agreements with vendors, and periodic risk assessments.

Business associate agreements

Where required, we enter into business associate agreements (BAAs) with covered entities and other business associates. Employers, health plans, and healthcare organizations working with Denizen may request a BAA by contacting us.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
